Heres a simple worksheet I put together for a class my wife and I were asked to teach at our current church we serve at.  I hope this helps someone.

If you look at the Locations & Times section it says that we were hoping to start services in mid-March. Well, the planned location has decided to opt-out and not allow us to have services. So, we’re looking for a location. Also, we’ve approched several area locations but they were either out of our price range or where unreceptive to host a ‘religious organization.’
